Hospital discharge

While in hospital it is important to think about the next steps for you or your loved ones recovery and what care or support might be needed. 

We have a dedicated team who are based in the Atrium of the RUH, Monday to Friday, 10am until 3pm.  They can advise you on the support available.  If you need to speak to a member of the team, please call 0300 247 0050.  

The team also support patients and their loved ones at Paulton and St Martin's Community Hospital. The Ward staff will put you in touch with them.

Advocacy support for BME patients

Support for patients in hospital and within the community to ensure they have access to advice, advocacy, and resources so their views are heard and their needs are met.

Advice and support for patients to claiming benefits, managing their money and debt.  Services also include information and advice on how to save money on energy bills

Services are available to support patients to improve their health and wellbeing focussing on stop smoking, achieve a healthy weight and being more active.

Hoarding / de-clutter

Identified as part of discharge where a safeguarding referral has been made by SWAST or another professional has identified the need for intervention to take place prior to discharge.

Home Independence Support (formerly Home from Hospital)

The Home Independent Support service provides practical and emotional support on discharge and includes shopping, domestic support and welfare checks to help prevent a readmission for 4 to 6 weeks.  The service is also available for people who are at risk of being admitted to hospital.

Home fire safety visits

Avon Fire & Rescue help people who may be more at risk of having a fire in their home and offer a free pre-arranged visit. Would you benefit from a Home Fire Safety visit, that includes smoke or CO2 alarms, fire safety equipment, cooking, planning escape routes.

BANES patients only that are:-

  • Aged 65 years or over
  • Sight and/or hearing loss
  • Mobility concerns
  • Mental health concerns
  • Memory concerns
  • Unable to escape unaided if there is a fire
  • Family with children under the age of 5 years

Home repairs

Identified as part of discharge by ward staff, where a patient’s heating or hot water system is not working or repairs at home are deemed unsafe for the patient to return home.  Assessment needs to be made about the patients current living environment before agreement to carry out the works.  Consent by the patient is required.

Referrals will be made to Curo where a patient is a Curo Tenant

Homeliness and tenancy support

Support for patients in hospital.  The team will visit patients on the ward to look for solutions to either end or prevent homelessness. 

Support is also provide for patients who need help to look at their housing options, housing benefits, managing mortgage and rent arrears or tenancy advice.

Curo also support patients who are Curo tenants that need help with repairs, to live more independently, rent arrears.

Mental health support

Bath Mind provide support for patients in hospital and as they prepare to leave hospital.  Teams will visit patients on the ward to offer support.

Support from the CWH Connectors to help a patient consider options for a pendant alarm to help them maintain their independence.

Specialised support for health conditions

A wide range of services that support for patients, family and carers.  Services offer a range of support for the following conditions:-

  • Are facing a life-limiting illness
  • Recently had a stroke
  • Are living with Dementia
  • Have a cancer diagnosis

Unpaid carers

Services to support family or friends who make undertake an unpaid caring role:

  • information, advice and support around their caring role
  • help to create an emergency plan if the carer is not available for the patient they care for
  • offer advice on financial aid, including potential grant and funding opportunities
  • free wellbeing activities and peer support activities

Wellbeing activities and social groups

A wide range of services to support patients to access various wellbeing groups and activities after recovery.  These include:-

  • people who are 55 and over to access social groups, memory groups, volunteering, befriending and digital inclusion
  • for the BME community to access social groups, trips and day services
  • connecting with local social networks and signposting to wellbeing groups and activities.
