About us

We work together as a partnership, bringing statutory, commissioned, and contracted services into one place. By joining forces under one roof, we make it easier for people to access support they need.  

By combining expertise and resources, we provide person-centred services that support a wide range of needs—from staying healthy and active to housing, financial advice, social connection, and reducing isolation.

Our story 

In March 2020, six partners came together to provide vital support during the Covid-19 pandemic, along with the incredible efforts of over 2,000 community volunteers recruited by 3SG, together, we were able to respond quickly and effectively to those in need.  Since then, our partnership has grown and evolved, strengthening our commitment to deliver person centred care.

Our aims

  • Co-ordinate services: bringing together services to support a person's health and wellbeing and wider determinates of health.
  • Access and connect to help: a single place for people to access help without needing to make multiple contacts.
  • Focus on prevention and early intervention: focus on reducing crises by addressing issues early.
  • Community engagement: encourage collaboration between professionals, volunteers, and residents to build resilience and social connections.

We have produced a number of videos and case studies to demonstrate the positive impact of integrated and person-centred support.  They showcase how co-ordinated services address complex needs - such as isolation, complex health needs, financial needs, caring for a loved one - while leveraging digital innovation to enhance accessibility and efficiency.

Ageing Well

In 2024, we commissioned an evaluation of our Ageing Well service.  The report explored how our multi-disciplinary team worked and collaborated to offer a holistic support for people and their families.  Read the report below and watch the video to find out more.

Digital Transformation 

Supporting hospital discharge - a shared referral management system

In 2022, we worked together with the Royal United Hospital to create a digital solution designed to assist a two-way conversation between hospital staff and patients to assist discharge. 

We created a simple online referral form to help patients put in place the support they need as they prepare to go home from hospital.  The form focuses on areas of need, with priority given to discharge dependent services delivered by CWH partners.  A dashboard view enables ward staff to keep up to date with the progress of referrals and third sector partners are able to work together to deliver a more co-ordinated approach to care.  Read more in our case study and watch the video to find out more.

MJ Awards recognition

CWH recognised at the MJ Awards public private partnership

In June 2025, we were recognised for our partnership working at the annual Municipal Journal Awards.  We're delighted to have been acknowledged with a highly commended award for innovative partnership working to support thousands of residents in B&NES. The MJ Awards celebrates excellence in UK local government. It recognises the achievements of individuals, teams, and organisations that have made a significant positive impact on their communities.
