CWH in hospital
Our Community Wellbeing Hub team is based in the Atrium of the RUH. We can advise you on the support available as you or your loved ones prepare to leave hospital. We also support patients and their loved ones at Paulton and St Martin's Community Hospital. The Ward staff will put you in touch with them.
Preparing to leave hospital
While in hospital it is important to think about the next steps for you or your loved ones recovery and what care or support might be needed. We can help, find us in the Atrium of the RUH, There is no need to book an appointment, please drop by and meet the team between 10 and 3pm, Monday to Friday. If you need to speak to a member of the team, please call 0300 247 0050 or ask a member of staff in the hospital to connect you.
Discharge Referrals for patients
We make it easy for ward staff to make a referral. One contact can connect a person to multiple services. We have created a discharge form that enables Ward staff in hospital to discuss with each patient what community support they will need when they return home from hospital. If you are making a referral for an inpatient in the RUH, Paulton or St Martin's Community Hospital you can complete the discharge form. The form must be completed before the patient leaves hospital.
