Who do we support? Chevron

Our support is available to all residents across Bath and North East Somerset. We support individuals, families and carers too.

How does it work? Chevron

We reduce the number of calls you need to make. One contact to the Hub can connect you to multiple services delivered by our partners. We are a central point of access to a range of health and wellbeing services. You can find out more about our partners here or give us a call on 0300 247 0050.

How can I get support? Chevron

We know that asking for help can be the biggest barrier when people are feeling vulnerable and isolated. We try and make it as easy as possible to get the help needed.  You can contact us directly or you can ask someone to contact us on your behalf, including contact from friends and family and professionals. You will need to give consent before a referral can be made. Give our Triage Team a call on 0300 247 0050.

I want to contact the organisation direct, and not go through the CWH. Can I do this? Chevron

Yes.  It doesn't matter if you contact the organisation directly or through the CWH, we want you to get the right support, at the right time.

How do you work together? Chevron

We work as a team to support you in the best way we can, especially if you have more than one issue to deal with. You may hear from different partners offering different services, and you can opt out at any time. You may be contacted by a number of partners who are delivering different services.
