Managing at home

We offer a wide range of support to help people manage at home and maintain their independence. Services include practical help with daily tasks, advice on home adaptations and equipment, and links to health and social care support. We also offer guidance on using digital tools for remote health monitoring and staying connected, as well as information on transport options such as Blue Badge applications and community transport volunteer schemes.

Our support

Ageing Well

Specialist service - Multi-agency team supports people at home to address the common ‘frailty syndromes’ of falls, immobility, delirium, incontinence and side effects of medication.  For people aged 70 years and over.

Criteria
Led by HCRG Care Group including Age UK, Carers Centre, BEMSCA, WERN, Stroke Association, Alzheimer's Society.

At Home Services

The At Home service, is a paid for service run by Age UK that assist people to continue living independently at home.  If a person is anticipating outpatient treatment that may require at home support this service may be available for free via the Home Independence Support offer.

Home fire safety visits

Avon Fire & Rescue help people who may be more at risk of having a fire in their home and offer a free pre-arranged visit, including smoke or CO2 alarms, fire safety equipment, cooking, planning escape routes.  Criteria for the service includes:

  • Aged 65 years or over
  • Sight and/or hearing loss
  • Mobility concerns
  • Mental health concerns
  • Memory concerns
  • Unable to escape unaided if there is a fire
  • Family with children under the age of 5 years

Home repairs

We Care and Repair is a service that supports people who need home repairs.  The service offers surveying to identify defects and organise trusted tradespeople to undertake the work.  The service is a paid for service and available if you own your own home includes case related services, grants and self-funders.

The service helps adults to live independently and confidently at home for as long as possible. The service offers short-term, practical, and enabling support focused on prevention, wellbeing, and connection to community life.

The Stroke Association provides support to learn more about stroke and self-care, to build confidence in daily living and communication (aphasia) strategies.
