Welcome to the Community Wellbeing Hub's Discharge service
Referrals can be made for hospital in-patients as they prepare to return home. Support includes home from hospital, food support, help with benefits, mental health, housing and support groups. Services are delivered by partners of the Community Wellbeing Hub. PLEASE COMPLETE THE FORM BEFORE THE PATIENT LEAVES HOSPITAL.
