HCRG are responsible for delivery community health and care services across BANES, Swindon and Wiltshire.
Services include
- Care Co-ordination Centre
- Urgent Community Response (UCR) service
- Dedicated health visitors
- Specialist nurse care, eg heart failure, lymphoedema and intravenous therapy
- Community hospital inpatient wards
- Minor Injuries Unit
- Reablement
- A range of outpatient therapy services including physiotherapy
