Easier Access to NHS services

Community health services play an essential role in helping people of all ages stay healthy and live well.  Across Bath and North East Somerset, Swindon and Wiltshire, HCRG Care Group is responsible for community health services on behalf of the NHS. These services are for everyone, from birth through adulthood and into old age.

Across the region, partners from the health and care sector, local authorities and voluntary organisations are working together to make it easier for people to get the support they need to live well for longer.  From 1 April 2026, community health services for the NHS across the region will operate under the name Bath and North East Somerset, Swindon and Wiltshire Community Health.

What are community health services?

NHS community health services support people of all ages to stay well and live independently. You might get support from a nurse or therapist in your home, at a local group or clinic or through specialist services to help you recover after a hospital stay or manage ongoing health needs. 

From April, you’ll find all these services under Bath and North East Somerset, Swindon and Wiltshire Community Health.

Getting the right support

In many cases, you can refer yourself directly to community health services by phone or online, without needing to see a GP first. A team of Community Health Navigators will help guide you to the most appropriate support or information.

The phone line will be open 24 hours a day, seven days a week, with routine adult support available 8am–8pm, and dedicated support for children and young people 8am–5.30pm Monday to Friday (closed on Bank Holidays).

Online, you’ll also find easy access to health advice, self-help resources and information about local services at any time. Non-digital options will still be available for anyone who prefers or needs them.

Focusing on what matters to you

Professionals from health, therapy and wellbeing services will also work more closely together through Integrated Neighbourhood Teams, focusing on what matters most to you and your family.

These changes will make NHS community health services easier to access, bring more care closer to home and help people across the region live well for longer.
