Last year, B&NES Village Agents conducted a review of 100 clients that we had supported in one year. The results show the huge range of situations we come across. Our person-centred, face to face approach combined with a network linking us to professionals and local providers helps each person find the best solution. We are active partners in the Community Wellbeing Hub alongside at least 30 other organisations. Our commitment to this collaboration is absolute and all professional referrals now come to us through the secure RIVIAM referral management system. Most Wednesdays will find us attending the Hub MDTs for complex cases discussions. The exchange of information and suggestions ensure that each person has the best solutions offered without duplication and in a managed way that prevents overwhelm. In some cases where a person is already receiving help or may have disengaged, new approaches can be agreed.
