Our Community Wellbeing Hub team are based in the Atrium of the RUH. We can advise you on the support available as you or your loved ones prepare to leave hospital. There is no need to book an appointment, please drop by and meet the team. If you need to speak to a member of the team, please call 0300 247 0050.

Preparing to leave hospital

While in hospital it is important to think about the next steps for you or your loved ones recovery and what care or support might be needed. Referrals for professionals

The CWH On-Ward Admission form enables Ward staff in the RUH to discuss with each patient what community support they will need when they return home from hospital. If you are making a referral for an inpatient in the RUH you can complete our referral form below. The form must be completed before the patient leaves hospital.
