Our Partners
The Community Wellbeing Hub was set up in March 2020 and provides a central hub of wellbeing services delivered by a range of partner organisations for residents across Bath and North East Somerset. The Hub currently has 28 partners of which 23 are commissioned to deliver council and NHS services.
A Collaborative Approach
Our partners bring together a range of specialist skills and knowledge. A short triage process ensures that people can access the most appropriate services to meet their needs. If we identify a clinical or social care need, we can refer any required support to our professional colleagues.
The Community Wellbeing Hub is made up of a number of 'Pods' which are led by our partner organisations and supported by a team of people and a wider network of external organisations. Each 'Pod' is responsible for actively monitoring referrals within its RIVIAM Multi-agency Referral Hub Service.
